We are looking for a licensed LVN ready to lead the clinical heartbeat of our community - not just chart oversight, but the culture that makes guests feel truly known. As our Health and Wellness Coordinator, you'll be our LVN of record and the leader who holds our entire care team to a standard that goes beyond service.

• Medication oversight, guest health assessments, and care coordination as our community's LVN of record
• Direct supervision of our Resident Care Coordinator, Med Techs, and Resident Assistants
• Regulatory readiness across our Type A and Type B licensed community, including memory care with Alzheimer's certification standards
• A team you'll train, coach, and hold to a standard of individualized, guest-specific care - not generic programming

• Real ownership - you shape both the clinical operation and the culture of the community you help lead
• A community that trains its whole team in guest-first, individualized care, not just the nursing staff
• Direct partnership with the Executive Director on staffing, hiring, and performance decisions for your team

• Clean HHSC surveys on both the general health-services standards and the Alzheimer's certification requirements
• A community where staff can describe specific, individualized things they do for guests - not generic programming
• An active, current Roman Candle Log with evidence it's actually used, not just filled out
• Low turnover on our care teams and strong family satisfaction scores
• A nursing operation that runs cleanly enough that you spend the majority of your time on guests and staff, not paperwork recovery

• Active, unencumbered Texas LVN license
• 2+ years of nursing experience in senior living
• Prior leadership or supervisory experience in a Texas licensed assisted living or memory care setting preferred
• Certified Dementia Practitioner (CDP), or willingness to obtain one within your first 6 months
• Comfort holding both a clinical scope of practice and a leadership role - you won't be purely administrative here
• Ability to be on your feet for extended periods and move throughout the community multiple times per shift
• Ability to respond to guest emergencies, including physical assistance consistent with the community's safe guest-handling policies
• Availability for on-call rotation and occasional evening or weekend coverage for community, clinical, or family needs
• Availability for weekend/holiday manager on duty responsibilities
