We are seeking a Full-Time Licensed Practical Nurse (LPN) to join our Home Health & Hospice team serving Grand Island and surrounding communities
. This rewarding position allows you to provide personalized nursing care while working closely with patients, families, physicians, and an interdisciplinary care team dedicated to improving quality of life.
As a member of our Home Health & Hospice team, you will:
- Deliver skilled nursing care in patients' homes
- Monitor patient conditions and communicate updates to the healthcare team
- Assist with implementing individualized care plans
- Educate patients and families on health conditions and care needs
- Provide comfort-focused care and support to hospice patients and their loved ones
- Promote patient independence, safety, and overall well-being
Qualified candidates will have:
- Current, active Nebraska Licensed Practical Nurse (LPN) license
- Current CPR certification
- Strong clinical assessment and communication skills
- Compassionate, patient-centered approach to care
- Excellent organizational and time-management abilities
- Ability to work independently while collaborating effectively with a team
- Commitment to providing exceptional service and care
