Nurse Program Coordinator: Headache clinic and Neurology infusion center

Summary:

The Nurse Program Coordinator is responsible for assessment, coordination of care, discharge planning, nursing care delivery, evaluation, and documentation of care for pediatric patients and their families.

Responsibilities:

• 1. Assesses patient/families physical, emotional, spiritual, psychosocial and learning needs; communicates pertinent information to the team and other health care providers; assess and recognize changes in patient condition and family dynamics.
• 2. Provides care coordination and care planning for target population
• 3. Provides nursing care to target population of patients and documents evaluation and care delivery in patient plan of care.
• 4. Determines the education needs of nursing personnel, multi-disciplinary team members and others; implements patient and family education in collaboration with the multi-disciplinary team.
• 5. Provides leadership to the nursing team and acts as a change agent to improve patient care.
• 6. Serves as a resource to other nurses and healthcare providers with expertise in assigned practice area.
• 7. Other duties as required.

• 2. Experience in communication, teaching skills, and problem-solving skills is required
• 3. Experience working with all levels within an organization is preferred
• 4. Experience in healthcare is required
• 5. Proficiency in MS Office [Outlook, Excel, Word] or similar software is preferred. Epic software is preferred.
