Licensed Practical Nurse (LPN) - Primary Care Dayton, Ohio Position Responsibilities

• Provide direct and indirect nursing support in the primary care environment.
• Obtain patient history and support assessment and care planning.
• Document care and report changes in condition to the RN or physician.
• Monitor patient response to medications and therapies.
• Assist in emergencies and support diagnostic or procedural care.
• Review labs and medical records as part of patient support workflow.
• Provide patient and family instruction.
• Participate in discharge planning and team-based care functions.
• Maintain professional conduct, patient privacy, and customer service standards.

• Graduate of an accredited practical nursing program.
• Current CPR certification from an American Heart Association vendor.
• Minimum one year of direct patient care experience within the last three years.
• VA experience preferred, not required.
• Ability to obtain and maintain VA credentialing and clearance requirements.

• Permanent Resident/Green Card holder with a Social Security Number and must have resided within the United States for the past three years;
• No sponsorship available.
