LVN Home Healthcare

Aveanna Healthcare is seeking an LVN for adult home healthcare patients. In this role, you'll provide skilled nursing care in patients' homes, including assessments, wound care, medication administration, and chronic disease management. You'll follow individualized care plans, document accurately, and collaborate with RNs, physicians, and families. • Provide skilled nursing care to adult patients in their homes
• Perform comprehensive patient assessments and monitor vital signs
• Administer medications and treatments per physician orders
• Provide wound care and chronic disease management
• Document care accurately in electronic health records
• Follow individualized care plans and report changes in condition
• Collaborate with RNs, physicians, and interdisciplinary team members
• Educate patients and families on care, medications, and safety
• Maintain infection control and patient safety standards
• Communicate effectively with supervisors and participate in training
