Nurse Practitioner - Population Health - Exempt

The Population Health APP blends direct patient care with community health promotion, chronic disease prevention and management, and data-driven quality initiatives. This role focuses on optimizing health outcomes, closing health equity gaps, and managing care delivery for defined patient groups—particularly vulnerable or underserved populations. The APP works autonomously and collaboratively within interdisciplinary care teams to implement value-based care strategies and preventative programs;
Provides medical care services within the APP’s scope of practice and consistent with the nurse’s education and certification as defined by the Ohio Board of Nursing, as well as in accordance with the privileges granted by the Board of Trustees. The Population Health APP provides care to patients, independently and collaboratively, as stated in the Standard Care Arrangement with the collaborating physician(s).

Prescribes drugs and therapeutic devices in accordance with Section  of the Ohio Revised Code, and assesses, plans, implements, and evaluates appropriate plan of care including diagnostic and therapeutic interventions in both the clinic and patient home settings, as well as in the inpatient setting.

Requirements Include

• Current RN licensure to practice nursing in the State of Ohio.
• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
• National board certification in an applicable population focus (Family Nurse Practitioner [FNP] or Adult-Gerontology Primary Care Nurse Practitioner [AGPCNP]) via the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
• Palliative Care designations and/or board certification through The Hospice and Palliative Credentialing Center (HPCC), The Center to Advance Palliative Care (CAPC) or equivalent.
• Clinical background in primary care, community health, or population health; familiarity with value-based care metrics and electronic health record (EHR) data analytics.
• Current Standard Care Arrangement with collaborating physician(s).
• Current American Heart Association (AHA) Healthcare Provider CPR (BLS) and/or Advanced Cardiac Life Support (ACLS) certification.
• A minimum of 3 years of clinical nursing experience in critical care, complex care management, palliative care or hospice care required; 5 years preferred.
