Nurse Practitioner; CNP; Palliative Medical Practice, PRN

, you’ll step into patients’ lives during some of their most vulnerable moments, helping them navigate serious illness with clarity, comfort, and dignity.

This is a field-based role in Albuquerque where you’ll provide home- and community-based care, working with a collaborative interdisciplinary team while maintaining a high level of autonomy in your daily practice. • Conduct comprehensive palliative care assessments addressing physical, psychosocial, and spiritual needs
• Manage complex symptoms such as pain, dyspnea, nausea, and fatigue using evidence-based approaches
• Lead goals-of-care conversations, including advance care planning and end-of-life decision support
• Collaborate closely with physicians, social workers, chaplains, and other team members to deliver coordinated care
• Evaluate and facilitate appropriate transitions to home health or hospice services
• Document care plans and clinical decisions accurately and in compliance with regulatory standards

• Active, unrestricted Nurse Practitioner license in New Mexico
• MSN or DNP from an accredited program; board certification (FNP, AGPCNP, or equivalent) required
• Prior hospice experience at some point in your nursing career
• Strong clinical judgment and confidence managing complex symptom burden
• Ability to navigate emotionally sensitive conversations with empathy and clarity
• Excellent communication and collaboration skills across interdisciplinary teams
• A patient-centered mindset and the ability to work independently with accountability
