Hospice RN Liaisons
to join our team!

In this role, you’ll meet with patients and families who are considering hospice care — providing clinical assessments, answering questions, and helping them navigate an important decision with confidence and comfort. You’ll also build strong relationships with hospitals, referral sources, and community providers to ensure a smooth and supportive transition into hospice services.

This is a great opportunity for a nurse who enjoys both clinical care and relationship-building. You’ll also assist with admissions when needed and play a key role in ensuring patients receive compassionate, high-quality end-of-life care.

• Current Registered Nurse (RN) license in the state of Pennsylvania
• Minimum of two [2] years nursing experience
• Marketing/Public Relations experience preferred
• Admission and discharge planning experience preferred
• Knowledge of hospice and palliative nursing preferred
