Registered Nurse – On-Call

Trellis Supportive Care is seeking a compassionate and skilled Registered Nurse – On-Call to provide exceptional nursing care and support to hospice patients and their families.

As an On-Call RN, you’ll play a vital role in ensuring patients receive timely, compassionate care when they need it most. You’ll assess patient needs, triage calls, make home visits when indicated, manage symptoms, collaborate with the interdisciplinary team, and help patients and families navigate end-of-life care with dignity and support.

Schedule:

Sunday-Thursday 4p-12midnight (40 hours per week)

What You’ll Do

• Provide compassionate nursing assessment, symptom management, treatment, education, and follow-up according to the patient’s plan of care.
• Triage patient and family calls and make home visits based on clinical assessment or patient/family needs.
• Utilize strong clinical skills in areas such as wound care, lab draws, and PCA pumps.
• Identify changes in patient condition and collaborate with physicians and the interdisciplinary team to ensure appropriate interventions.
• Provide emotional support and education to patients and families related to disease progression, symptom management, and end-of-life care.
• Maintain accurate clinical documentation and ensure compliance with hospice, regulatory, and agency standards.
• Collaborate with the interdisciplinary team to address patient needs and support quality hospice care.

What You’ll Bring

• Current North Carolina Registered Nurse (RN) license.
• Current CPR certification.
• Bachelor’s Degree in Nursing preferred.
• Minimum of one year of clinical nursing experience preferred; homecare nursing experience strongly preferred.
• Strong assessment, critical thinking, problem-solving, organization, and communication skills.
• Understanding of hospice philosophy and the unique physical, emotional, and psychosocial needs of patients and families.
• Valid driver’s license and required automobile liability insurance.
