# Per Diem APRN - FNPs Needed Hot Job## Overview Level Experienced Position  Type Per Diem Job Shift Day Education  Level Graduate Degree Category Health  Care## Description Regional Hospice and Palliative Care is looking for per diem Advance Practice Nurses with hospice and palliative care experience for our inpatient Center for Comfort Care and Healing (12 bed hospice residence in Danbury).  

Candidate may also work with our Home Hospice (adult and pediatric) and Palliative Care programs, doing  face to face  patient assessments and collaboration with interdisciplinary team regarding patient's plans of care. Our Home Hospice and Palliative Care programs serve patients throughout Fairfield, Litchfield and New Haven counties.

Primary needs are rotating weekend availability
** RESPONSIBILITIES:
*** Screens patient medical status and ensures Medical Director or designee approval prior to admission.
* Manages clinical care, including medical orders, prescriptions upon discharge and discharge summaries as needed.
* Maintains accurate and comprehensive medical records
* Actively coordinates medical care with patient’s primary care provider
* Maintains all current certifications as well as membership in professional organizations
** Additional Responsibilities may include but not limited to:
*** Face to face evaluation of home hospice patients
* Participate in weekend and overnight on-call as needed## Qualifications
* Must have clinical expertise, current nursing CT license, and ability to interface with medical community
* Current CT APRN license required; FNP helpful.
* Home care/hospice experience required, advance hospice and palliative care (ACHPN) credentials preferred
* Pediatric hospice and/or palliative care experience desired
* Current DEA number required
* Current NPI number required
* Valid driver’s license required.
* Position requires good written and oral communication skills, with ability to work in variety of environments
