Weakley Nursing & Rehabilitation Center is seeking a detail-oriented and clinically skilled Licensed Practical Nurse (LPN) to serve as our Medical Records & Infection Prevention Coordinator
. This unique leadership role combines clinical oversight, regulatory compliance, infection prevention, and medical records management to help ensure exceptional resident care and survey readiness.

Key Responsibilities Medical Records- Maintain accurate, complete, and confidential resident medical records.
- Ensure documentation complies with federal, state, and facility regulations.
- Monitor chart audits and documentation trends for quality and compliance.
- Assist nursing leadership with record requests, audits, surveys, and regulatory reviews.
- Maintain electronic and paper medical record systems.
- Support admissions, discharges, and resident record management processes.
- Lead and coordinate the facility's Infection Prevention and Control Program.
- Monitor, track, and analyze infection surveillance data and trends.
- Conduct infection investigations and implement corrective actions when needed.
- Educate staff on infection prevention practices, PPE usage, and isolation procedures.
- Ensure compliance with CDC guidelines, CMS regulations, and facility policies.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Collaborate with nursing leadership, physicians, and interdisciplinary team members to promote resident safety.
- Current Tennessee LPN license in good standing.
- Long-term care, skilled nursing, rehabilitation, or infection prevention experience preferred.
- Knowledge of infection control regulations and healthcare compliance standards.
- Strong organizational, documentation, and record-management skills.
- Excellent communication and leadership abilities.
- Ability to maintain strict confidentiality and professionalism.
- Experience with electronic medical record systems preferred.
