The Denver Hospice has an immediate opening for an RN Case Manager. The RN Case Manager oversees and coordinates patient and family care in accordance with hospice philosophy and the delivery of care, providing skilled nursing, intervention, documentation, supportive care and assessments for assigned patients.
This full-time role reports to the Interdisciplinary Team and requires travel daily; licensure and CPR are required, with BSN preferred.
