Lily Care Hospice in Albuquerque, NM is hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up. You will manage a caseload in private homes and facilities, complete assessments, develop plans of care, provide pain and symptom management, and coordinate with the attending physician and Hospice Medical Director.
The role requires EMR documentation and participation in on-call rotations, with a Monday–Friday schedule
