A PDS Case Manager under Kentucky’s HCB (Home and Community Based) waiver helps Medicaid waiver participants direct their own in‑home services while staying safely in the community.

1099 Position - Pay will be discussed in interview

Core

Job Duties
  • Develop and maintain a person‑centered Plan of Care for each participant, including waiver and PDS services, and update it at least annually or when needs change.
  • Monitor services through required home visits (often monthly for PDS) and phone/telehealth contacts to ensure health, safety, and goals are being met.
  • Assist participants to use Participant Directed Services help hire, train, and manage their own caregivers, and maintain all required employee paperwork and contracts.
  • Review Medicaid eligibility and coordinate reassessments so the person stays eligible for the waiver level of care.
  • Enter data and case notes into the Medicaid Waiver Management Application (MWMA) and keep complete electronic and paper files.
  • Monitor budgets, timesheets, and Electronic Visit Verification (EVV) to be sure services match the Plan of Care and meet state rules.
  • Advocate for participants with providers and community resources (home care agencies, DCBS, DAIL, DMS, etc.).
  • Example:
    You might visit a participant monthly, review their hours used with their PDS caregiver, update goals, fix EVV clock‑in issues, and request a change in hours if needs increased.
Skills And Abilities
  • Strong organization and time‑management to handle a large caseload, deadlines, and detailed documentation.
  • Ability to work independently in the field and as part of a team with nurses, finance, and state agencies.
  • Good communication and teaching skills to explain waiver rules and train participants and caregivers.
  • Comfort using computers and web‑based systems (MWMA, EVV, electronic records).
  • Ability to manage stressful situations, problem‑solve, and protect participant health and safety.
Education And Requirements
  • Typically, a bachelor’s degree in social work, human services, psychology, sociology, nursing, or related field; some agencies accept other degrees plus human‑services experience or may hire RNs with relevant experience.
  • Valid driver’s license and reliable transportation for home visits.
  • TB test and CPR/First Aid, often required within a few months of hire.
  • Clear background checks and any trainings required by the waiver program.
Work Environment and Schedule

Full‑time position, usually weekday business hours, with some flexibility; many postings offer hybrid or partial work‑from‑home for documentation days.

Travel within a region (for you, often within a multi‑county area in Kentucky) to complete home visits.

Documentation, calls, and coordination are often done from home or office between visits.
