OVERVIEW:
The Outreach, Triage and Intake Coordinator (OTIC) serves as the primary entry point to the Enhanced Care Management (ECM) program, coordinating outreach, referral development, engagement, triage, intake, assessment, enrollment, and initial care planning for eligible children, youth, and families. The OTIC builds collaborative relationships with community partners and referral sources, conducts comprehensive assessments, determines program eligibility, develops initial care plans, and facilitates timely enrollment into ECM services using trauma-informed, culturally responsive, and family centered practices.
DUTIES:(30%) Outreach and Community Engagement Develop and maintain collaborative relationships with Managed Care Plans, healthcare providers, behavioral health agencies, schools, child welfare agencies, probation, community-based organizations, and other referral partners.
Conduct outreach activities in community settings, schools, clinics, shelters, and community events to identify and engage potential ECM participants.
Educate community partners, families, and stakeholders regarding ECM services, eligibility requirements, referral pathways, and available resources.
Coordinate outreach presentations, informational meetings, and stakeholder engagement activities.
Create and distribute outreach and marketing materials consistent with agency and contract requirements.
Participate in community collaboratives, networking events, and cross-system meetings and internal case conferencing meetings to strengthen referral pathways and partnerships.
Promote culturally responsive and equitable access to ECM services.(35%) Triage, Assessment, and Intake Services Receive, review, and respond to referrals in accordance with ECM and Managed Care Plan requirements.
Conduct triage screenings to determine eligibility, level of need, urgency, and appropriateness for ECM and other community services.
Complete intake assessments to identify strengths, needs, risk factors, protective factors, and service priorities.
Conduct assessments utilizing approved tools including, but not limited to, CANS, ANSA, risk assessments, crisis assessments, social determinants of health screenings, and other program- required instruments.
Assess for immediate safety concerns and coordinate crisis intervention services when necessary.
Gather and review relevant medical, behavioral health, educational, and social service information to support care planning.
Complete intake documentation, informed consent forms, releases of information, enrollment packets, and required program records.
Provide orientation to ECM services and explain participant rights, responsibilities, and program expectations.
Collaborate with children, youth, families, caregivers, and referral sources to identify service goals and priorities.
Utilize clinical judgment to determine service needs and make recommendations regarding services, resources, and enrollment.(20%) Enrollment, Care Planning, and Administrative Responsibilities Develop initial care plans based on assessment findings and identified needs.
Facilitate enrollment into ECM services and coordinate warm handoffs to assigned Lead Care Managers.
Coordinate referrals and linkage to healthcare, behavioral health, educational, housing, and community resources.
Document all outreach, engagement, triage, intake assessment, enrollment, and care planning activities in accordance with Medi-Cal, CalAIM, Managed Care Plan, and agency requirements.
Maintain accurate and timely documentation within the electronic health record.
Participate in supervision, staff meetings, trainings, case consultations, and program development activities.
Maintain accurate records and reporting related to outreach activities, referral sources, enrollments, and program outcomes.
Ensure compliance with HIPAA, Medi-Cal, CalAIM, and agency documentation standards.
