Job Summary
The Family Liaison serves as a supportive partner to children, youth, and families throughout planning, implementation, transition, and aftercare processes. Using strength-based and family-driven approaches, the Family Liaison assists families with system navigation, resource connection, advocacy, skill development, and implementation of individualized family-centered plans. Working closely with the Wraparound Facilitator, family team members, and community partners, the Family Liaison supports family goal achievement, continuity of care, transition planning, and structured aftercare services while promoting family voice, family choice, and long-term success.
SpecificResponsibilities
- Partner with families to identify strengths, needs, goals, and priorities; support implementation of family-driven plans; and serve as an active member of Wraparound, Family Group Decision Making (FGDM), and other family-centered planning teams.
- Assist families with system navigation, resource identification, advocacy, and access to behavioral health, educational, healthcare, social service, and community-based supports.
- Collaborate with the Mii Wrap Facilitator, family members, natural supports, and service providers to support action planning, goal implementation, skill development, and follow-through on strategies identified through family-centered planning processes.
- Reinforce, practice, and help families generalize skills and strategies developed through Wraparound, FGDM, and other family-centered interventions to promote long-term success and stability.
- Support development and implementation of transition and closing plans and provide structured aftercare and follow-up services to strengthen continuity of care and maintain connections to supports following service completion.
- Promote family voice, family choice, cultural responsiveness, and family-centered practices while fostering trusting relationships that empower families to actively participate in decisions affecting their lives.
- Develop and maintain collaborative relationships with schools, county agencies, healthcare organizations, mental health providers, community organizations, and family-run organizations to strengthen support networks and resource access.
- Maintain accurate documentation, referral records, progress notes, and required program records while ensuring confidentiality and compliance with agency, state, and federal requirements.
- Participate in supervision, team meetings, trainings, quality improvement activities, and other duties as assigned.
- High school diploma or equivalent required. Experience supporting children, youth, and families through professional, community-based, or lived experience is preferred. Relevant education, training, certification, or experience in human services, family support, behavioral health, education, or a related field is beneficial.
- Knowledge of children s mental health, family support services, community resources, and educational systems. Ability to build trusting relationships, support family voice and choice, communicate effectively, and work independently and collaboratively with families and community partners.
- Commitment to culturally responsive practices and the ability to empower and support families in achieving goals, navigating systems, and accessing resources.
- Valid Minnesota driver s license, reliable transportation, and automobile insurance required. Ability to work flexible hours, including occasional evenings, as needed.
Starting Salary:
This is a full-time position with comprehensive benefits. Salary will range from $49,412 annually depending on experience, education and training.
Travel:
Extensive travel within a five-county area and occasional travel within the state and nationwide.
