Job Description Summary

Responsible for helping patients and their families manage health or life problems. This position assesses patients for psychosocial and economic barriers that interfere with healthcare goals. This role works closely with case managers, physicians, nursing staff, and spiritual support to provide counseling services, crisis intervention, stress management, and continuity of care. In addition, this position addresses patient and parent coping, bereavement, and issues of abuse and neglect.

Advocates for patients and families to ensure their needs are met.

Job Description
  • Performs psychosocial assessments and provides psychotherapy to patients and families coping with medical issues.
  • Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community.
  • Collaborates with the multi‑disciplinary team to establish and document psychosocial determinants for safe discharges.
  • Serves as a consultant and educator to the multi‑disciplinary team regarding the psychosocial components of illness, disability, and hospitalization.
  • Facilitates staff emotional debriefing and ongoing support groups. Applies knowledge of local, state, and federal programs to assist with appropriate referrals and coordination of post‑acute care.
  • Participates in the development of programs to support patients and families dealing with complicated medical issues.
  • Attends physician rounds and forums to facilitate communication and clinical social work input to the interdisciplinary team.
  • Demonstrates working knowledge of the involuntary commitment process. Provides counseling services, crisis intervention, stress management, and bereavement support.

Social Work from an NASW accredited university

Work Experience
  • 2+ of position‑related experience [Required]
  • 2+ work [Required]
  • Of direct clinical with case management experience preferable in a health or mental health setting with an emphasis on mental health, substance abuse and/or geriatric population [Required]
