Job Scope:
The Social Worker will provide case management, counseling, and resource coordination for shelter clients.
This includes conducting needs assessments, developing individualized service plans, and connecting clients with appropriate human service support.
They facilitate group sessions to address the different needs of clients while contributing to ending homelessness in Bronx Family Network Inc. and New York City.
- Provide support and guidance for clients and the overall operations of Bronx Family Network shelters.
- Focus on clients' needs and implement suitable crisis intervention resolution when needed.
- Conduct and complete assessments upon admission and regularly review and update care plans.
- Developing and implementing intervention plans to address client needs and goals.
- Must be able to independently assess the psychosocial functioning and needs of clients to formulate and implement a treatment plan, identifying the client’s problems, strengths, weaknesses, coping skills, and assistance needed.
- Must have knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
- Track clients' progress, and compliance with DHS guidelines, and monitor housing placements.
- Work with other services and conduct case conferencing, with parties such as outreach workers, teachers, doctors, and law enforcement, to provide comprehensive support to the client.
- Coordinate services and monitor clients’ progress to ensure clients move from shelter into permanent housing and receive the necessary social service support that is needed.
- Collect and compile data in accordance with DHS/BXFN standards.
- Enter all appropriate case notes and data in the DHS/CARES database system.
- Performs related tasks as assigned.
- Master’s degree in social work required from an accredited Institution.
- CSW or LMSW license from the New York State Department of Education is essential and must be obtained within one year of appointment.
- Familiarity with local housing resources, public benefits, and community-based services.
- Excellent communication, organizational, and time management skills.
- Proficiency in Microsoft Office and Case management software (e.g., CARES).
- Bilingual (English/Spanish) Preferred.
This position operates in an agency office located in a shared and open workspace. The position uses standardized office equipment such as phones, computers, fax machines, and copiers.
Work Environment:This role operates in a shelter or multi-site setting and requires frequent on-site presence. It also occurs in an office setting in a shared and open workspace. The use of standard tools such as phones, fax machines, copiers, computers, and other equipment is essential.
PHYSICAL DEMANDS:- Ability to sit and/or stand for long periods of time.
- Ability to walk the building and go up the stairs in the event of an emergency.
- Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes.
- Vision adequate to observe video surveillance, read correspondence, computer screens, forms, etc.,
- Ability to travel using public transportation to other BXFN offices or other facilities.
Works collaboratively with interdisciplinary teams to ensure integrated support services. Maintain respectful and professional relationships with clients, staff, and community partners.
RESIDENT, CLIENT, GUEST, AND STAFF FOCUS:Maintain a trauma-informed, client-centered approach. Support residents' emotional, social, and psychological well-being. Promote empowerment and autonomy while ensuring the safety and stability of the clients and people who they serve.
