Job Scope:
As a Case Manager working with Bronx Family Network in a DHS shelter within NYC, your role is to provide comprehensive support to residents, including needs assessment, case management, and crisis intervention.

The Case Manager also plays a crucial role in connecting clients with community resources and advocating for their well-being.

As a Case Manager, you will be responsible for setting clear, measurable goals and fostering a supportive, empathetic relationship with clients based on trust and accountability. You will help clients address their needs and transition to permanent housing through direct service or referrals.

Duties and Responsibilities
  • Conduct assessments to determine client needs, develop individualized Independent Living Plan (ILP), and provide ongoing case management support.
  • Conduct weekly/bi-weekly ILP with clients, and document meetings in CARES within 48 hours.
  • Conduct weekly/bi-weekly unit inspections and document/upload into CARES.
  • Check and respond to work-related emails daily to ensure timely communication and awareness of important updates, assignments, and organizational notices.
  • Assist and accompany clients with housing search, viewings, job search, opening their PA case, and documenting in CARES.
  • Maintain written and oral communication of incidents per policies in CARES.
  • Ensure accurate and timely documentation of client information, including intake forms, progress notes, and other relevant records.
  • Connect clients with appropriate community resources, such as housing, healthcare, and social services, while advocating for their needs.
  • Educate clients on topics such as budgeting, landlord-tenant relations, and healthy living practices.
  • Adhere to shelter policies, DHS and OTDA regulations, and reporting requirements.
  • Work closely and effectively with shelter staff and other service providers and clients to ensure coordinated care.
  • Manage a caseload of 20-25 cases and keep accurate and up-to-date records of all cases assigned.
  • Assist client with referral to medical/ human services provider in the community.
  • Performs general administrative functions such as answering phones, and emails, completing reports, and professionally serving food.
  • Report any problems to the shelter director.
  • Perform other related duties as assigned or requested by the shelter director.

Qualifications
  • Bachelor’s degree in a relevant field (social work, psychology, or human services) from an accredited Institution.
  • Minimum 2 years of Case Management Experience
  • Experience working with homeless or underserved populations.
  • Familiarity with community resources
  • Familiarity with local housing resources, public benefits, and community based services.
  • Proficiency in Microsoft Office and CARES case management software.
  • Excellent oral and written communication, organizational, and time management skills.
  • Bilingual (English/Spanish) preferred.

This position operates in an agency office located in a shared and open workspace. The position uses standardized office equipment such as phones, computers, fax machines, and copiers.

PHYSICAL DEMANDS
  • Ability to sit and/or stand for long periods.
  • Ability to walk the building and go up the stairs in the event of an emergency.
  • Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes.
  • Vision adequate to observe video surveillance, read correspondence, computer screens, forms, etc.,
  • Ability to travel using public transportation to other BXFN offices or other facilities.

Collaborate with co-workers, supervisors, and interdepartmental staff to align with the company's mission and improve operations.

RESIDENT, CLIENT, GUEST, AND STAFF FOCUS

Respond to the practical, emotional, and spiritual needs of residents and staff. Adjust priorities for emergencies and keep supervisors informed of important developments.
