Hourly Rate: $16.90 - $26.23 per hour
Position SummaryThe Support Worker provides monitoring, support, training, and encouragement to individuals with disabilities who live in their own homes or are preparing to transition into their own homes. This position supports individual goals, daily living needs, community access, health and safety, and independence while maintaining respectful, person-centered services in accordance with HCAR policies and program expectations.
Essential Duties and ResponsibilitiesEssential duties and responsibilities include the following. Other duties may be assigned based on program and organizational needs.
- Implement objectives identified in consumers' Individual Support Plans and provide support consistent with assigned service needs.
- Provide monitoring, encouragement, and skill-building support related to daily living, independent living, social participation, housing stability, and community access.
- Support consumers with emotional support, nutrition awareness, house finding, budgeting, shopping, household management, and money management skills.
- Assist consumers in accessing community resources, appointments, services, and other supports as appropriate.
- Provide transportation to appointments, community activities, errands, and assigned service locations when required and approved.
- Maintain accurate and timely daily consumer support notes, service documentation, and progress information.
- Communicate with the Program Director, case managers, Regional Center representatives, service providers, family members, and other agencies as appropriate.
- Attend staff meetings, program meetings, and required training as assigned.
- Assist consumers with self-help and personal care activities, including feeding, toileting, dressing, hygiene, and other support needs assigned and trained.
- Assist consumers with special devices, including wheelchairs, walkers, prostheses, braces, or other adaptive equipment as needed.
- Lift, transfer, or assist adult consumers as required using proper training, safety procedures, and assigned support plans.
- Submit semi-annual progress reports and other required documentation related to consumer services and support needs.
- Identify and report health, safety, behavioral, environmental, or service concerns to the appropriate supervisor in a timely manner.
- Maintain confidentiality of consumer information and follow HCAR policies, safety practices, mandated reporting requirements, and applicable program procedures.
- Perform other job-related duties as assigned to support consumers, program operations, and organizational needs.
This position does not provide direct supervision of employees.

Minimum Requirements- High school diploma or GED required.
- Minimum of six months of related experience working with individuals with disabilities is preferred.
- College coursework in social welfare, sociology, psychology, education, social services, or a related field is desirable.
- Knowledge of community resources and the ability to support consumers in accessing appropriate services and support.
- Ability to read and understand safety rules, service plans, documentation requirements, operating instructions, and organizational procedures.
- Ability to complete routine reports, progress notes, correspondence, and required service documentation accurately and on time.
- Ability to communicate clearly and professionally with consumers, support teams, HCAR staff, Regional Center representatives, other agencies, and community members.
