• Conduct comprehensive assessments of residents' social, emotional, and behavioral health needs to inform personalized care strategies
• Schedule and attend weekly care plan meetings
• Utilize motivational interviewing techniques to motivate residents toward positive change and goal achievement
• Proven experience in social work and long term care, with a focus on behavioral health.
• Demonstrated success in crisis intervention and crisis management within community or clinical settings
• Prior experience in addiction counseling or substance abuse treatment programs preferred
  
  Strong communication skills with the ability to build trust and rapport with diverse populations
