The Institute for Health and Recovery (IHR) is a statewide service, research, policy, program development and capacity building agency. IHR designs its services based on an understanding of the impact of trauma. IHR’s CARF accredited treatment programs provide holistic interventions for hard-to-reach and high-risk individuals and families. IHR’s mission, since our founding in 1990, has been to develop and support a comprehensive continuum of care for families, individuals, youth, and pregnant and parenting people affected by alcohol, tobacco, and other drug use, violence/trauma, mental health challenges and other health issues, while advancing principles of health equity and social justice.

Our work is focused on addressing and eradicating inequities in our health care, economic, and justice systems, especially as they disproportionately affect women and people of color. Summary Statement :
The Clinical
Case Manager is an experienced Master’s level staff person who will work directly with youth in IHR’s Malden TIP (Teach Inspire Prepare) Program, an innovative elementary and middle school-based program (grades 4-8) and other youth programs. The Clinical Case Manager will work closely with the Case Management team, including clinical supervisor, case manager, and school personnel, to deliver targeted prevention and intervention programs to students identified as “high risk” and demonstrating concerning behaviors in school and the community.

The Clinical Case Manager will also provide home and community-based services to support youth involved with the Department of Children and Families.

Essential Functions:
  • Provide clinical case management services to youth enrolled in Malden’s Salemwood School.
  • Deliver strengths-based services to youth involved with the Department of Children and Families.
  • Outreach to enrolled youth and their families including Salemwood personnel, caregivers, state agencies, and additional referral sources to provide program information.
  • Conduct intakes and needs assessments for program participants.
  • Provide case management services to Salemwood students and their families using strengths-based and trauma-informed approaches.
  • Engage in care coordination with families, school personnel, community providers, state agencies and other collaterals.
  • Develop, implement, and review Individual Service Plans review based on program requirements .
  • Proactively identify, and make referrals to, existing community resources to support various life domains
  • Lead the process to plan and facilitate after school and summer activities for participants and their families.
  • Provide individual mentoring to participants and lead developmentally appropriate activities.
  • Lead development, maintenance, and updating of community resources lists to ensure timely and appropriate referrals.
  • Co-facilitate psychoeducation groups that promote life skills training and strategic peer involvement.
  • Support family service efforts through family education and family nights.
  • Maintain and adapt schedule based on needs of programming, including after business hour availability to facilitate youth and family activities.
  • Actively work with school personnel to provide education advocacy, tutoring and homework support.
  • Utilize incentives to engage clients in program activities.
  • Regularly collaborate with other IHR staff as well as key partners including Salemwood staff, DCF, and community-based providers.
