School Based Community Case Manager

We are seeking a driven, adaptable, and compassionate professional to serve as a School-Based Case Manager within our Opportunity Academy,an alternative school setting supporting7th-12thgradestudents with complex behavioral, emotional, and environmental needs.

The ideal candidate brings a trauma-informed approach to every interaction and is passionate about helping vulnerable youth succeed both in and out of the classroom.

Provide direct case management services to students identified with behavioral health needs.

Develop, implement, and monitor individualized service plans in collaboration with students, families, and school staff.

Support students in building coping skills, emotional regulation, problem-solving, and social skills.

Maintain consistent contact with students to ensure engagement and progress toward goals.

Respond to behavioral and emotional crises within the school setting using de-escalation and trauma-informed strategies.

Provide immediate support and stabilization during high-risk situations.

Collaborate with school staff and clinical teams to ensure student safety and appropriate follow-up care.

Starting pay: $24.15/hour

Working with youth (e.g., Behavioral Health Technician, Teacher, Para-Educator, Youth Worker).

Previous case management experience preferred; experience working with children (ages 2–18) is highly helpful.

Crisis & Behavioral Management: Strong de-escalation and crisis management skills; comfortable working with and setting professional boundaries for children with mental illness.

Adaptability: Ability to think quickly, problem-solve, and pivot priorities in high-pressure situations based on the needs of students and the organization.

Approach: Committed to a trauma-informed approach when working with high-risk, vulnerable youth and diverse populations.

Professionalism: Proven ability to build rapport while maintaining firm professional boundaries; strong interpersonal and communication skills.

Technical Proficiency: A solid foundation in computer skills is required.

Licensure: Must possess a valid driver’s license and be insurable.

Compliance: Ability to pass extensive background checks (including DCF).

Flexibility: Willingness to maintain a flexible schedule as daily needs vary based on patients and school requirements.
