POSITION OVERVIEW Reentry+Clinical):

The Clinical Case Manager provides trauma-informed, clinically-oriented case management services to adults with serious mental health conditions who are transitioning from incarceration, probation, or other justice system involvement back into the community.

This role integrates clinical assessment, care coordination, and reentry stabilization services, supporting clients in navigating the complex challenges of reintegration, including housing instability, behavioral health needs, substance use recovery, and access to medical and social services.

The Clinical Case Manager applies clinical judgment, risk assessment, and crisis intervention skills to promote safety, reduce recidivism, and support long‑term recovery. Services are delivered in the field—including homes, correctional reentry settings, and community agencies—as well as via telehealth.

Working as part of a multidisciplinary team, the Clinical Case Manager collaborates with probation/parole, behavioral health providers, housing partners, and community-based organizations to ensure coordinated, client-centered care. This role will report directly to the Program Supervisor.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
  • Conduct comprehensive clinical and reentry-focused assessments, including mental health, substance use, criminogenic risk factors, housing stability, and psychosocial functioning.
  • Perform and document mental status examinations (MSEs) and ongoing risk assessments, including evaluation of safety concerns, relapse risk, and recidivism factors.
  • Develop and implement individualized, recovery-oriented service plans that address both clinical needs and successful community reintegration goals.
  • Provide intensive, field-based case management to support clients during critical transition periods following release from incarceration or institutional settings.
  • Deliver crisis intervention, de‑escalation, and safety planning, coordinating higher levels of care (e.g., hospitalization, detox programs) when clinically indicated.
  • Support clients in accessing behavioral health treatment, including linkage to psychiatric services, therapy, and substance use recovery programs.
  • Collaborate with probation/parole officers, reentry programs, correctional facilities, and legal system partners to ensure compliance with conditions of release while supporting client rehabilitation.
  • Assist clients with securing stable housing, including transitional housing, reentry housing programs, shelters, and independent living placements.
  • Facilitate access to benefits and entitlements (e.g., SSI/SSDI, Medi-Cal, Cal Fresh), identification documents, and employment or vocational services.
  • Provide rehabilitation counseling and skill‑building interventions, including life skills, coping strategies, and community integration support.
  • Coordinate care with medical and dental providers, ensuring continuity of care for co‑occurring physical health conditions.
