Location: Northern
CVS Health is seeking a Field Social Worker to support DSNP members in Charleston, WV. You will perform home visits, assess needs, set goals, and coordinate with providers and community resources.
This full-time role requires travel 50-75% and a Bachelor of Social Work (MSW preferred). You will advocate for members to access Medicaid, Medicare, and plan benefits while partnering with care teams to improve outcomes.
