Location: Northern
PATH is seeking a Case Manager for the Metro Villas site in Los Angeles, California. The role supports clients through housing journeys from street outreach to permanent housing, coordinating health, social services, and benefits while building individualized plans.
As part of the SHS team, you will conduct intakes, assess needs, provide referrals, and maintain documentation. This full-time, non-exempt position requires driving and a commitment to ending homelessness.
