* Summary:

** The Case Manager provides comprehensive, client-centered case management and care coordination services to neurodivergent adults and their families in Broward County served through Joshua’s Path Disability Services. This position involves providing services in office, home, and community settings, along with coordinating interdisciplinary care and connecting clients to appropriate evidence-based services and community resources.

The Case Manager works independently to assess client needs, determine eligibility for services, develop individualized service plans, and coordinate support across medical, behavioral health, educational, and community systems. This position serves as a liaison between clients, families, and service providers to ensure timely, effective, and compassionate care delivery. In addition, the Case Manager may support the Joshua’s Path team by assisting with group activities, community outings, and other participant-centered events, contributing to program engagement and the overall participant experience.
** Responsibilities:
**** Case Management & Care Coordination
*** Manage a caseload of clients and families, providing comprehensive case management services.
* Conduct psychosocial and clinical assessments to identify client and family needs.
* Determine eligibility for Joshua’s Path services and external programs at the local, state, and federal levels.
* Develop, implement, and monitor individualized service plans aligned with evidence-based practices.
* Coordinate interdisciplinary care from intake through ongoing service delivery.
* Monitor client progress and update care plans based on measurable outcomes.
** Client & Family Support
*** This position involves providing services in office, home, and community settings
* Educate clients and families on diagnoses, services, and available interventions.
* Offer ongoing guidance, emotional support, and systems navigation.
* Advocate for access to services, benefits, and community resources.
** Service Coordination & Referral
*** Maintain a network of community providers and referral sources.
* Facilitate referrals to healthcare providers, schools, early intervention, employment, residential programs, and social services.
* Collaborate with multidisciplinary teams to ensure holistic client care.
* Act as a liaison between families and service providers to ensure continuity of care.
** Home- and Community-Based Services
*** Conduct in-home visits to assess safety, well-being, and environmental factors impacting care.
* Identify barriers such as housing instability, safety risks, or lack of resources.
* Provide field-based support to promote independence and quality of life.
* Respond to crises, including behavioral or environmental emergencies, and coordinate appropriate interventions.
** Documentation & Compliance
*** Maintain accurate, timely, and confidential electronic client records in accordance with agency and regulatory standards.
* Complete case notes, assessments, service plans, and reporting documentation per and grant guidelines.
* Track and document service utilization and outcomes.
** Collaboration & Professional Engagement
*** Participate in case conferences, staff meetings, and supervision sessions.
