Internal Role Description (NYC Region)
Position OverviewThe ICAM Case Manager provides individualized, participant-centered support to help New Yorkers successfully enter, persist in, and complete healthcare education and training programs. This role focuses on identifying and addressing the unique socio-economic obstacles found in the five boroughs, supporting career pathway development, and connecting participants to the resources and relationships needed to achieve long-term success in the NYC healthcare workforce.
The Case Manager serves as a consistent point of contact, working closely with Program Managers and regional partners and community organizations to promote engagement, retention, and completion.
Key Responsibilities1. Participant Support and Case Management
- Provide ongoing, one-on-one support to participants navigating healthcare education and career pathways.
- Support participants in defining goals and developing Individualized Action Plans that account for NYC-specific cost-of-living and scheduling challenges.
- Build trusting relationships that support consistent engagement, motivation, and persistence through challenges.
- Monitor participant progress and proactively identify risks to retention or completion.
2. Barrier Identification and Wraparound Support
- Assess non-academic barriers with a focus on NYC-specific systems, including Fair Fares/MTA navigation, NYCHA/Section 8 housing stability, and HRA childcare vouchers.
- Agency Navigation:
Coordinate referrals and “warm handoffs” to borough-specific CBOs; assist participants in navigating the Access HRA portal and local SNAP/cash assistance systems. - Digital Equity Advocacy:
Identify and address “Digital Divide” obstacles, helping students access low-cost broadband and the hardware necessary for clinical coursework (e.g. laptops, chrome books, accessibility aids, etc.) - Follow established processes for documentation, approvals, and tracking of wraparound supports in alignment with program and fiscal requirements.
3. Career Pathway Navigation and Workforce Development
- Support participants in understanding healthcare career pathways, training requirements, and employment opportunities.
- Assist participants through key transition points, including program entry, clinical rotations, and job placement.
- Collaborate with education providers and employer partners to align participant goals with local workforce opportunities.
4. Recruitment and Community-Based Engagement
- Support recruitment through regional networks and community partnerships.
- Assist in outreach and engagement efforts with education providers, workforce partners, and community organizations.
- Participate in information sessions and community events across the five boroughs to build program visibility.
- Provide real-time feedback on recruitment effectiveness and barriers to enrollment.
5. ICAM System Utilization and Resource Alignment
- Utilize the ICAM platform to document participant engagement, services provided, and outcomes.
- Maintain accurate, timely, and detailed case notes and records.
- Contribute to the organization of region-specific non-academic support resources (borough-by-borough resource guides).
- Support alignment between ICAM resources and case management workflows.
- Support mentor recruitment through regional networks.
- Provide input into mentor matching based on participant needs and career interests.
- Assist with coordination and ongoing feedback loops between mentors and participants.
- Contribute to mentoring program refinement and continuous improvement.
