Overview

The Residential Program Coordinator is responsible for the overall management and day-to-day operations of an OMH-funded supportive housing program serving individuals with serious mental health needs and/or histories of homelessness. The Manager provides leadership and supervision to program staff, ensures residents receive appropriate supportive services, oversees clinical and housing-related documentation, and maintains compliance with OMH regulations, contractual requirements, and agency policies.

The position requires strong leadership, clinical documentation, crisis-management, communication, and organizational skills, with a focus on promoting resident stability, independence, wellness, and successful community living.

Job Details

Essential Functions:
  • Oversee the daily operations of the supportive housing program and ensure effective delivery of services to residents.
  • Ensure program activities are consistent with OMH regulations, contractual requirements, agency policies, and best practices.
  • Monitor program performance, service delivery, resident outcomes, vacancies, incidents, and required program data. Including monitoring of the Single Point of Access system (SPOA) for referrals and update accordingly.
  • Coordinate with landlords and Catholic Health attorneys for tenant leases.
  • Submit monthly rent file and complete patient rental worksheets annually and as needed.
  • Maintain a safe, supportive, trauma-informed, and respectful residential environment.
  • Coordinate with agency leadership and other departments to address operational and resident-related needs.
  • Supervise, support, and provide ongoing guidance to case manager and other program personnel.
  • Conduct regular individual supervision, staff meetings, performance evaluations, and training.
  • Monitor staff performance and documentation to ensure services are provided consistently and appropriately.
  • Assist with staff recruitment, onboarding, training, and professional development.
  • Provide guidance and support to staff managing complex resident situations and crises.
  • Oversee the delivery of individualized supportive services designed to promote housing stability, independent living, recovery, and community integration.
  • Complete, review, and update OMH treatment plans for community residents
    , ensuring that plans accurately reflect residents' mental health needs, strengths, goals, progress, and service interventions.
  • Collaborate with residents, case managers, clinical staff, and other service providers to develop individualized treatment goals, measurable objectives, and appropriate interventions.
  • Monitor resident progress toward treatment-plan goals and revise plans as needed based on changes in residents' needs, functioning, or circumstances.
  • Ensure treatment plans, progress notes, assessments, and related clinical documentation are completed accurately and within required time frames.
  • Ensure clinical and program documentation meets OMH, contractual, regulatory, and agency standards.
  • Advocate for residents' needs and assist with connections to mental health services, healthcare, benefits, employment, education, and community resources.
  • Promote resident choice, self-determination, recovery, independence, and quality of life.
Housing Stability
  • Monitor residents' housing stability and address issues that may place tenancy at risk.
  • Work collaboratively with residents and staff to address rent arrears, lease violations, behavioral concerns, conflicts, and other housing-related issues.
  • Coordinate with landlords, property managers, housing providers, and community agencies regarding resident and property concerns.
