Roof Above’s services and programs reach more than1,400 individuals every day across nine campuses in Charlotte.
Roof Above was born through the merger of Urban Ministry Center and Men’s Shelter of Charlotte in 2019.
Roof Above owns and will begin operating during the summer of 2025 a substance-free transitional housing community at Kings on Lamar (KOL). This campus will provide transitional housing for working men, in addition to being the site for our Veterans housing program (Clinical and GPD). This property consists of 2 buildings and 64 rooms.
POSITION SUMMARYThe Case Manager(s) of Kings on Lamar assist employed and/or employment seeking men who have experienced homelessness by providing support services in a transitional housing program setting. The case manager(s) will engage with program participants to ensure basic needs are met while exploring access to appropriate community services needed such as medical and mental health care, employment opportunities and any other relevant support.
ESSENTIALFUNCTIONS
- Provide case management services to help participants successfully transition from homelessness to housing. Manage a case load of approximately 20 individuals.
- Support participant(s) by providing assistance with resume building, completing employment and appropriate benefits applications, referral to mental health and/or medical services, etc.
- Conducts in office meetings, room visits with participants several times a month to assist with stabilization.
- Assist and provide support around life skill development (ie, financial literacy, stress management, transportation etc).
- Assess needs and ensure that program participants are linked to appropriate specialty services.
- Provide ongoing evaluation of any presenting symptoms and assess need for supportive services. Link program participants to internal and external resources.
- Respond appropriately to participant emergencies, conflicts, and behavioral concerns. Use crisis prevention and intervention techniques to handle acute incidents of physical and mental health crisis and aggression.
- Ensures participants fully comprehends and remain compliant with established community guidelines.
- Become certified in Homeless Management and Information Services database to enter services and view records, as needed. (Certification will be provided).
- Maintain program participant records, including assessments, notes, statistical/demographic information, and specific, achievable and measurable participant-centered plans in NC Homeless Management Information System.
- Perform other duties as assigned.
- Bachelor’s degree in human services field or equivalent experience.
- Minimum of two years case management experience as well as experience with adults who experienced homelessness and substance use.
- Experience in supportive housing, homeless services, other affordable/subsidized housing, preferred experience with sober-living and substance use recovery
- Ability to work independently and collaboratively with clients as well as service providers and landlords.
- Ability to handle and prioritize multiple tasks.
- Self-starter with excellent problem-solving and crisis management skills
- Requires a valid driver’s license
