Location: Northern
CVS Health is seeking a Field Social Worker to support dual eligible DSNP members across a broad geography. You will conduct home and community visits, assess needs, set goals, remove barriers, and coordinate care with providers and care teams.
This role requires LMSW/LCSW in Pennsylvania, 2+ years of medical social work experience, a valid driver s license, and the ability to travel up to 75% of the time. A base salary is offered with benefits and bonus eligibility.
