** Community Support Specialist**, you will work directly with individuals experiencing

* Help clients
** find, secure, and maintain housing
*** Build and maintain relationships with
** landlords and housing partners
*** Assist with
** housing applications, resources, and financial assistance options**### Case Management & Advocacy
* Provide
** community-based case management
** to individuals in supportive housing
* Connect clients to
** healthcare, employment, education, and social resources
*** Advocate on behalf of clients to ensure access to needed services### Skill Building & Recovery Support
* Help clients build
** independent living skills** (budgeting, hygiene, daily routines)
* Support development of
** coping skills and recovery strategies
*** Provide education on
** mental health, substance use, and wellness**### Collaboration & Care Coordination
* Work closely with
** treatment teams, healthcare providers, and community partners
*** Participate in
** treatment planning and team meetings
*** Coordinate services to support long-term success and stability### Documentation & Follow-Up
