** Position Summary**:
Coordinates and schedules daily homeless outreach activities across both street and rural environments. This role serves as a primary point of contact for community partners, conducts field based coordinated assessments, and represents the agency at regional provider meetings. The coordinator is also responsible for precise data entry, tracking outreach across multiple databases.

* Principle Duties and Responsibilities:

** 1. Conducts regular outreach to unhoused and/or unsheltered community members, including street outreach, and rural outreach. Settings such as shelters, community courts, libraries, food banks, encampments, meal sites, treatment and recovery centers, and others as required.
2. Conducts outreach in a manner that promotes safety, confidentiality, respect for client dignity, and upholds SNAP’s mission and values. Always maintains regular communication with supervisor while in the field.
3. Empower participants to achieve economic and personal independence and self-sufficiency by providing case management services following assessment tools.
4. Conducts homeless coordinated assessments and assists in determining eligibility for supportive housing activities.
5. Completes housing inspections in alignment with habitability standards and requirements specific to each funding source.
6. Ensure files on homeless clientele are accurate and updated, including data entry into multiple databases. Maintains required case and program records as required in an accurate and timely manner.
7. Works collaboratively with other team members and providers to assure continuity of services.
8. Assists households to access mainstream resources including public assistance, social security, food stamps, Medicaid, and employment services. Provide referrals if needed.
9. Participates in activities such as coalitions, committees, and task force meetings to plan and develop resources to address community needs.  Represents SNAP on appropriate community committees and boards and with the media as approved.
10. Attends scheduled staff meetings and training.
11. Works with coworkers and volunteers to promote a positive, respectful, and productive working environment, while protecting client confidentiality and dignity.
12. Within the context of the stated principal job duties, serve as an ambassador of SNAP’s mission and values.
13. Completes assigned projects and performs related duties as assigned or as the situation dictates.
14. Always provides excellent customer service as well as helps to grow the customer service culture within the agency.## Qualifications

* ** Education and Experience
**
* Required:

Three years’ experience in homeless outreach, case management, social services or related field; or three years of progressively responsible experience in the social services field. Must have clean driving record and valid driver’s license.
* Preferred: AA Degree with three years’ experience in homeless outreach, case            management, social services, or related field.# Knowledge, Skills, and Abilities
* Ability to empathize with and advocate for individuals with diverse backgrounds and needs, including those from low-income households
* Demonstrated communication, life management and motivational skills to promote self-sufficiency rather than supporting dependence
* Ability to work with various assessment and planning tools
