About Us:

McGregor PACE (Program of All-inclusive Care for the Elderly) We are a comprehensive, community-based healthcare program that helps older adults remain safely and independently in their homes and communities. PACE brings medical care, social services, therapy, behavioral health, nutrition, transportation, home care, and other supports together through an interdisciplinary team.

We are seeking passionate and dedicated Community Resource Liaison’s to join our growing team.

Job Description:

As a Community Resource Liaison, you will serve as a vital link between the participants in the PACE program and available community resources. You will provide information about services, accessing services and resources they need to remain healthy, safe and independent as possible in the community.

Reports to:

The social worker. The community resource liaison works as part of the interdisciplinary team under the direction of the MSW/LSW.

Key Responsibilities:
  • Coordinate and follow up with community agencies, healthcare providers, social service organizations and other service partners.
  • Support participants with housing, social support, and other needed services.
  • Help identify barriers to care and communication concerns to the MSW/LSW.
  • Assist with scheduling and coordinating community-based services as directed.
  • Maintain accurate, timely and confidential participant records and documentation.
  • Perform other duties as assigned by the MSW/LSW.
Qualifications:
  • Previous experience in community outreach or social services is highly desirable.
  • Strong interpersonal communication and writing skills.
  • Knowledge of local community resources and services.
  • Ability to work independently and as part of a team.
  • Computer skills and knowledge of Microsoft Office Suite .
