- Carbondale, CO 81623## Overview Salary Range $50.00 - $60.00 Hourly## Description As a PMHNP with Jaywalker Lodge, you’ll have the opportunity to do more than manage symptoms—you’ll help men build a foundation for lasting recovery. You’ll work alongside an experienced, multidisciplinary team to provide psychiatric care for clients navigating substance use, trauma, and co-occurring mental health conditions in a supportive, community-based setting.

RESPONSIBILITIES:
* Conduct psychiatric evaluations to include intake assessments as well as follow-up assessments (
** approximately 3 days onsite, 10 to 15 hours total per week)
*** Create, communicate, and carry out treatment plans with the patient and patient’s families
* Prescribe medications when necessary
* Communicate with patients regarding diagnosis and treatment plan
* Make referrals to specialists as appropriate
* Provide services at community partnership location
* Other duties as assigned## Qualifications
* Must possess an unrestricted license to practice in the state of Colorado
* Must possess an unrestricted DEA license in the state of Colorado
* Must have a current board certification by a national certifying body as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
* Eligible for paneling on all major insurance plans and to participate in Medicare
* Excellent communication, patient relations, time management skills, and safety awareness are required
