Job Summary

Pay: From $27.47 per hour.
Position: Intensive Case Planner (ICP).
Division: Behavioral Health Care.
Status: Full Time with Benefits.
Location: Community Based Response.
Base

Schedule:

35-hour work week, Monday – Friday 8:30‑4:30, with occasional nights, weekends and on‑call rotation.

• Maintain a caseload of families with elevated service needs and/or high‑risk indicators.
• Conduct regular home and community‑based visits (minimum twice per month or as mandated).
• Engage children and families using a trauma‑informed and culturally responsive approach.
• Complete comprehensive family assessments and individualized service plans in accordance with DFCS.
• Coordinate services including mental health, substance use, domestic violence, education, housing, and employment supports.
• Facilitate family team conferences and case planning meetings with internal and external stakeholders.
• Maintain timely and accurate documentation in electronic case records (e.g., Connections).
• Submit all required reports, progress notes, and court reports in accordance with DFCS and agency guidelines.
• Ensure all contacts, consents, and assessments meet regulatory and agency standards.
• Collaborate with DFCS, legal representatives, and service providers to ensure coordinated care.
• Coordinate with clients and treatment providers to secure placement for children requiring secured detention.
• Attend and provide testimony at Family Court proceedings when required.
• Advocate for services that strengthen family functioning and promote child well‑being.
• Respond promptly to emergencies, including safety concerns, hospitalizations, or child removals.
• Work collaboratively with supervisors to assess risk and implement safety interventions.
• Participate in individual and group supervision.
• Attend case conferences, training sessions, and team meetings.
• Maintain certification or proficiency in evidence‑based models and/or mandated programs (Mandated Reporting).
• Maintain a professional manner at all times and adhere to the policies and procedures of the Agency.
• Perform various related tasks as assigned or required.
• Ensure punctual and regular attendance; report to work as scheduled and prepared.
• Be available to work occasional evening and weekend events as needed.
• Be available to participate in rotating on‑call schedules; respond to urgent matters outside regular hours.

• A Bachelor’s Degree in Social Work, Nursing, Psychology or a closely related Human Service field.
• Two (2) years relevant experience working with children/adults with mental health/behavioral health concerns.
• A valid New York State Driver’s License and reliable transportation required to conduct home/community‑based visits.
• Knowledge of DFCS Preventive Services model and evidence‑based interventions.
• Familiarity with state and local child welfare regulations and systems.
• Proven strong organizational, assessment and communication skills.
• Experience securing cooperation of and working effectively with individuals and agencies in the community.
• Strong engagement, assessment, and crisis intervention skills.
• Ability to build trust and maintain boundaries with clients from diverse backgrounds.
• Excellent organizational and time‑management skills.
• Strong written and verbal communication.
