We are seeking a talented Community Case Monitor II to join our team of nearly 500 employees. CCSI is contracted by Onondaga County to provide program management services, including specialized staffing. The Community Case Monitor II will be located in Onondaga County and have a primary responsibility of monitoring all aspects of Enhanced Voluntary Agreements (EVA) as well as AOT Court Orders as necessary.

The position will carry a caseload and provide support and oversight for high-needs individuals with serious mental illness who have a history of medication non-compliance and who live in Onondaga County. The Community Case Monitor II will coordinate and facilitate multi-disciplinary teams that include: the client, behavioral health treatment providers, case management, housing supports, and other community based organizations as outlined in the treatment plan.

The Community Case Monitor II will support management to mentor Community Case Monitor I staff on community resources and relationships. Community Case Monitor II will assist in supporting the evaluation process for individuals residing in the community. This role will be responsible for tracking, data collection, tool creation and assigned parts of reporting process. Supervision will be provided by the Forensic Case Monitor Supervisor, in close collaboration with the AOT Coordinator and Director of Forensic and Outreach Services.

CCSI is partnering with Onondaga County  to hire a/an Community Case Monitor II. This position is employed by CCSI and working on behalf of Onondaga  this role, you will
* Mentor and support Community Case Monitor I informally and formally in addressing complex client needs and develop relationships with community providers.
* Provide physical and technical assistance to the completion of evaluation process for individuals in the community.
* Assist Forensic Case Monitor Supervisor in collecting, documenting, and analyzing monthly and quarterly data within the AOT program.
* Initiate case reviews.
* Support program level data development, tracking, and reporting.
* Assess needs of referred and assigned individuals.
* Coordinate the development of system-wide treatment and support plans to include treatment services, case management, housing, transitional support services and other identified supports.
* Monitor client’s participation in and compliance with an established treatment plan.
* Schedule, plan, and facilitate team meetings.
* Communicate effectively with the client, providers, families, and criminal justice personnel to assist with developing and sustaining the plan.
* Create a sustainable treatment team by assigning roles and tasks to the treatment team to prepare for the transition to a lower level of care.
* Monitor treatment implementation and coordinates action when treatment, service, and support plan needs adjustment.
