Case Manager — Middletown, OH | SOS Wellness
SOS Wellness is seeking case managers to support people receiving outpatient behavioral health care in Middletown and the surrounding Middletown–Dayton corridor. This is a part-time or full-time 1099 contract opportunity.
The role- Provide community psychiatric supportive treatment (CPST) and therapeutic behavioral services (TBS) where appropriate.
- Coordinate with therapists and psychiatric providers, connect clients to community resources, and document care in the practice’s clinical system.
- Support people and families with practical barriers to care in a collaborative outpatient team.
Experience in case management or community behavioral health is useful. Relevant Ohio credentials, such as LSW, LPC, CDCA, or qualified behavioral health provider registration, are preferred. Tell us about your background in your application.
