Nexus Family Healing is seeking a Case Manager to join the Kinship team in our St. Cloud, MN office, serving relative/kinship caregivers. This on-site role focuses on bridging families with services, performing home studies, and advocating for culturally aware support.
You will network with community partners, conduct outreach, and maintain ongoing communication with families, while connecting kinship households to needed resources and licensing pathways.
