Important Application Requirements for Substitute Teachers in Oldham County Schools

• Professional Certified Staff Application – include 2 references (online).
• Valid Kentucky Teaching Certificate – upload with your application or provide to Personnel Office.
• Official College Transcript(s) and/or High School Diploma – upload with your application or provide to Personnel Office.
• Medical Examination and TB Risk Factor Assessment – complete and submit.
• Authorization to Release Information – will be sent electronically.
• Employment Eligibility Verification (Form I-9) – complete Section 1 only; attach a copy of your driver's license and social security card.
• Child Abuse or Neglect Check – CANS checks must be requested online; $10 fee; upload a copy of driver's license, birth certificate, or social security card.
• Form W-4 and K-4 – will be sent electronically.
• School Preference Form – will be sent electronically.
• Automatic Deposit Form – will be sent electronically; also need a voided check.
• Electronic Materials Code of Ethics Agreement – complete and provide.
• Form SSA-1945 – will be sent electronically.
• FBI Fingerprint Check – cost $54; schedule appointment as instructed.
• TRS Retirees (Only) – TRS members who retired on or after January 1, 2020 must complete the KTRS Reemployment Certification.
