Located in Grand Rapids, our beautiful Breton Woods Campus—one of two continuing care campuses—offers a rare part - time opportunity for a Licensed Practical Nurse (LPN) seeking schedule stability, role variety, and meaningful resident connections.

This unique position supports the Breton Extended Care Assisted Living (BEC) residence, offering a rewarding blend of clinical care, resident advocacy, and collaboration across multiple levels of care.

• Provide direct LPN care, including medication administration, treatments, assessments, vital signs, documentation, and timely communication of condition changes to physicians.
• Support daily clinical operations by assisting with staffing coverage, orienting new staff, and coaching team members under nursing leadership guidance.
• Monitor resident care needs in Assisted Living and Independent Living settings to promote quality care and regulatory compliance.
• Serve as a liaison among residents, families, physicians, and the interdisciplinary team to coordinate care and support admissions.
• Respond to resident emergencies and assist with physician procedures while delivering compassionate, resident - centered care aligned with organizational values.

• 2 Wednesdays per month

• Strong organizational and prioritization skills in a fast - paced environment
• Flexibility, resilience, and adaptability
• Comfort navigating multiple technologies and computer systems
• Excellent customer service and communication skills
• Ability to support daily Health Centre operations and collaborate across teams

• Graduate of an accredited LPN program with current Michigan LPN licensure
• Minimum of 2 years of nursing experience
• Previous experience in emergency room or home care case management strongly preferred
• Demonstrated strong assessment skills and ability to make independent clinical decisions
