The Social Worker, in collaboration with the clinical team, provides patient and family advocacy, discharge planning, and psychosocial intervention. The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources. When indicated, the Social Worker will assist with crisis intervention and legal reporting requirements.
Required EducationGraduation from an accredited School of Social Work;
Master’s degree in Social worker; current LMSW
CCM Certified
Required SkillsBasic computer and keyboarding.
Preferred SkillsMeditech Electronic Medical Record.
Required ExperienceThree years social work experience.
Preferred ExperienceExperience in the hospital setting.
