Help us help others:

The ICM/Wraparound Facilitator will engage high-need youth and their families in developing, implementing, and monitoring an intensive, individualized plan of care. This position helps the youth and families to identify a support team that will collaboratively design a plan based upon family needs, strengths, and goals. The ICM/Wraparound Facilitator will support the team as they implement the plan and monitor its success.

This position will require some early evenings. The position must have flexible scheduling, and travel is required.

You will get to:

• Coordinate and facilitate monthly Wraparound team meetings
• Provide leadership in the development, implementation, and evaluation of individualized plans using the phases and activities of Wraparound
• Implement the YES Waiver program and complete all required documents within the identified time frames
• Develop trust and mutual respect with youth and their teams
• Assist teams in identifying needs and creating a shared vision
• Assist families in identifying, recruiting, and engaging natural supports
• Enable teams to take ownership of the Wraparound process
• Support clients in identifying their strengths and building upon them
• Develop the problem‑solving skills and self‑efficacy of the youth and team members
• Identify barriers to full participation in the Wraparound process and develop strategies to overcome them
• Ensure the Wraparound process is completed with high fidelity to the model and track progress toward goals
• Provide support and advocacy for Wraparound teams
• Prepare summary documents and final reports
• Ensure that all required confidentiality and other compliance documents are current for each youth
• Educate families, providers, and the community about the Wraparound process
• Contribute to and support a positive, team‑oriented work environment
• Utilize Collaborative Documentation, and 95% of all documentation is entered within 24 hours
• Turn Service Request Forms (if applicable) within 24 hours, with a denial rate of less than 5%
• Provide at least 75% Person‑Centered Time each month or an average each quarter
• Promote and demonstrate Hill Country MHDD Centers' mission, vision, and values through both behavior and job performance on a day‑to‑day basis
• Effectively interact and communicate with Hill Country MHDD Center staff, persons in service, and their families/customers and partnerships
• Knowledge of or experience with the Wraparound Model
• Strong written and verbal communication skills
• Proficient in computer applications such as Microsoft Word or Excel
• Organized and effective multitasker

You’ll definitely need:

• Bachelor's degree from an accredited four‑year college or university in social, behavioral, or human services with the ability to credential as a Qualified Mental Health Professional.
• A valid Texas State Driver's License.
• Current automobile insurance and the ability to travel when necessary.
• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.

Preferred:

• One year of experience working with children and youth with serious emotional disturbance.
• Three years of experience working with children and youth with serious emotional disturbance, preferred.
