As a Long Term Care RN, you ll play a vital role in helping patients maintain their quality of life, regain independence, and achieve their healthcare goals. You ll work closely with patients, families, and an interdisciplinary care team to provide compassionate, individualized care in a welcoming environment.
In this role, you will:
- Assess, plan, implement, and evaluate patient care plans with our very experienced team
- Deliver skilled nursing care in accordance with professional standards, while working excellent patient ratios
- Collaborate with physicians, therapists, and care team members
- Delegate and coordinate care appropriately
- Maintain accurate clinical documentation
- Support quality improvement and patient safety initiatives
- Pursue ongoing professional growth and development

What We re Looking For- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license
- Basic Life Support (BLS) certification upon hire
- One year of clinical nursing experience preferred
- Geriatric, rehabilitation, or long-term care experience preferred
- Strong communication, critical thinking, and teamwork skills
- A commitment to delivering exceptional patient-centered care
