As a Long Term Care RN, you'll play a vital role in delivering exceptional patient care by:

• Assessing, planning, implementing, and evaluating individualized patient care plans
• Documenting care accurately and thoroughly within the electronic health record
• Collaborating with an interdisciplinary healthcare team to ensure the best patient outcomes
• Delegating care appropriately based on team members' education, credentials, and competencies
• Supporting patients and their families through compassionate, patient-centered care
• Participating in quality improvement, safety, and infection prevention initiatives
• Pursuing ongoing professional development and continuing education

• Graduate of an accredited School of Nursing
• Current Registered Nurse (RN) license
• Basic Life Support (BLS) certification upon hire
• Minimum one year of clinical experience as a Staff Nurse

• Bachelor of Science in Nursing (BSN)
• Specialty certification
• Experience in geriatric and/or rehabilitation nursing
