DaVita Kidney Care is expanding its clinical services to skilled nursing facilities (SNF) through DaVita Skilled Nursing Dialysis Services. The new service line supports dialysis patients residing in SNFs, with patients transported to dialysis three times weekly. Teammates will have access to DaVita resources to build the infrastructure and grow this service line. Ideal teammates have DaVita experience, thrive in a start-up environment, and are comfortable collaborating with cross-functional teams within the Village.
ResponsibilitiesProvide complete ongoing care for each patient to ensure dialysis treatment stays on track, working in conjunction with dialysis RNs.
Operate as a Full-Time PCT based at The Laurels of Huber Heights; may support additional local sites as needed. Schedule generally includes Monday–Saturday; specific days vary by market needs.
Deliver direct patient care in a safe, comfortable, hygienic dialysis setting.
Support complex care delivery with multitasking to meet clinical outcomes and the needs of patients and SNF partners.
Provide autonomous, critically thinking care: monitor patients before, during, and after dialysis; record statistics and observations; assist with hemodialysis machine setup.
Participate in DaVita training programs and complete training requirements.
May be asked to support other local SNFs as needed.
High school diploma or equivalent
OCDT certification required
Basic computer skills and proficiency in MS Word and Outlook
Training/Teaching/Healthcare Education background
