Due to an upcoming retirement, we are looking for an enthusiastic and confident Community Nurse Practitioner to join our friendly team.
The successful candidate will be passionate about supporting patients living with frailty and committed to providing outstanding care in patients own homes, ensuring their wellbeing in the later stages of life (with dedicated GP support from our Frailty Lead).
You must be a qualified nurse with excellent communication skills, as you will be liaising with a wide range of clinical and non-clinical teams, diverse patient groups, and their families.
The role is approximately 15 hours per week; however, this can be negotiated. Salary is also negotiable and dependent on experience and skills.
Main duties of the jobYou will assist medical personnel in the care of frail patients, including treatment, preventative care, screening, and patient education. Job responsibilitiesJOB TITLE:
Community Nurse Practitioner
MAIN PURPOSE OF POST:
To assist in the care of vulnerable patients that may be housebound or in a Care Home. The role will involve assessments, maintenance of care plans, health management and vaccinations. The role will require visiting patients using own vehicle.
RESPONSIBLE TO:
Nurse Manager and Assistant Managers
MAJOR
DUTIES AND RESPONSIBILITIES:
- Complete care plans for all new care home residents
- Complete care plans for all new patients added to CCT list.
- To update care plans on an annual basis (including creating and reviewing ReSPECT forms)
- To assist in any other vaccination programme as required
- Work with practice and care coordinators in respect of frail patients
- Request blood tests and take bloods.
- Advise patients on general health care and minor ailments with referral to general practitioners as necessary.
- Carry out physical examinations and observations when required
- Liaise with practice safeguarding team when required
- Refer patients to appropriate clinical or social departments/agencies
- Discuss social support
- Participate in the education and training of students of all disciplines and the induction of all members of practice staff with special responsibility for nursing staff.
- Based at Eskdaill, there will be a requirement to regularly visit patients in either their own homes or care homes
- There is a requirement to use personal transport to visit patients, and allowance will be paid based on mileage used. There will be a requirement for the vehicle to have business use on the insurance
- Maintain continued education including attendance at courses and study days as deemed useful or necessary for professional development.
- Maintain a written professional development plan and maintain an up-to-date portfolio which meets the requirements of registration with the NMC

- 5. Ability to self-motivate, organise and prioritise workloads.
- 6. Ability to supervise and co-ordinate the practice nurse team.
