The Physical Therapist Assistant (PTA) provides comprehensive skilled services under the direction of a Physical Therapist. The PTA educates patients and their family members, ensures patient safety, maintains accurate documentation, and stays compliant with all legal rules and regulations.

- Physical Therapist Assistant with a current state license
- Minimum one year PTA experience in an acute setting
- Home care experience desired
- Effective communication with patients, families, physicians, and coworkers
- Knowledgeable in therapy best practices and capable of making appropriate judgments
- Detail‑oriented, observant, and disciplined in work ethic
- Valid driver’s license and auto insurance with your name on the policy
- Provide skilled services at patients’ homes per the Physical Therapist’s plan of care
- Observe, record, and report patient reactions, changes, and progress
- Perform physical therapy procedures and diagnostic tests as medically prescribed
- Treat patients to relieve pain, restore function, and maximize performance using exercise, massage, heat, water, etc.
- Assess functional problems and coordinate interventions with the Physical Therapist to improve patient function
- Communicate effectively with physicians, therapists, and coworkers
- Participate in case conferences to promote coordinated care
- Recommend prosthetic/orthotic devices or adjust existing ones
- Educate staff on specific physical therapy modalities when appropriate
- Educate patients and families on therapy techniques, safety measures, and supportive activities
- Instruct proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and prosthetic/orthotic devices
- Encourage patient and family independence to prevent dependence on the agency
- Complete all clinical documentation per agency protocol and Medicare/Federal guidelines
- Conduct chart audits at agency location as requested and communicate scheduler changes
- Participate in on‑call requirements, case conferences, in‑service trainings, mandatory meetings, and orientation/training of new employees
- Drive to patient’s primary location per scheduled visit
- Maintain daily attendance at assigned visit locations
- Document all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24hours
- Document all aspects of start‑of‑care visits within 48hours
- Submit all required documentation within specified company deadlines
- Follow plan of care within the scope of practice for a Licensed Physical Therapist Assistant
