Monday‑Friday 8:30AM‑5:00PM, weekend on‑call/holiday as needed. Territory:
Preble and Montgomery County and surrounding areas.
Care and compassion are at the heart of what you’ll do as a Physical Therapist (PT). You’ll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. You’ll provide home‑based therapy care for patients as directed by the attending physician, interacting with patients’ families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey.
Home care provides a real‑world perspective about what really helps patients restore their health.
- Provide comprehensive skilled services as ordered by the attending physician.
- Educate patients and their family members and ensure patient safety.
- Document accurately and remain compliant with all legal rules and regulations.
- Perform OASIS assessments and develop individualized plan of care.
- Complete initial evaluation and admission visit within 48‑72 hours of referral.
- Develop and adjust plan of care as needed.
- Treat patients to relieve pain, develop or restore function, and maintain maximum performance using techniques such as exercise, massage, heat, water, etc.
- Promote continuity of care with appropriate admissions, transfers, and discharges.
- Counsel patients and families on disease/injury and how to manage in all phases of physical therapy.
- Instruct on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other devices.
- Supervise PTAs and nurse assistants, overseeing therapy services.
- Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
- Maintain active PT license in state of employment and comply with health requirements.
- Participate in on‑call requirements and case conferences, in‑service trainings, mandatory meetings, and orientation of new employees.
- Drive to patient’s primary location per scheduled visit.
- Physical Therapist with a current state license.
- Minimum one year of PT experience in an acute setting; home care experience desired.
- Effective communication skills and ability to create positive impressions with patients, families, physicians, and co‑workers.
- Knowledgeable in therapy best practices and able to make appropriate PT judgments.
- Detail‑oriented, observant, disciplined work ethic, and timely prioritization.
- Valid driver’s license and auto insurance with your name listed as a driver.
- Ability to drive to patient locations and meet physical position requirements.
- Guaranteed base salary plus an uncapped bonus structure.
