Physical Therapist (PT)

Agency: Kettering Home Care

Schedule: Weekend position, full time 3 days per week. Holidays per rotation.

Territory: Middletown, Somerville, West Elkton, Camden, Farmersville, Germantown, Gratis, New Lebanon, West Alexandria, Cedarville, Jamestown, Wilberforce, Xenia, Miamisburg, Carlisle, Franklin, Springboro, Riverside/Beavercreek, Kettering/Beavercreek, Centerville, Waynesville, Bellbrook, Spring Valley, Centerville.

How You'll Make a Difference

You will provide home‑based therapy care directed by the attending physician, interacting with patients and families while supporting their recovery journey.

How

You'll Work

Provide comprehensive skilled services as ordered, educate patients and families, ensure patient safety, and complete accurate documentation in compliance with legal rules.

Major Areas of Responsibility
  • Clinical Care & Treatment - Provide skilled PT in patient’s home; relieve pain, restore function; perform OASIS assessments; develop and adjust individualized care plans; complete initial evaluation within 48–72 hours; front‑load visits for high‑risk patients.
  • Communication & Education - Observe, record, and report patient progress to physician and agency staff; counsel patients/families; instruct proper use of equipment; promote continuity of care.
  • Documentation & Compliance - Complete clinical documentation per agency protocol and Medicare/Federal guidelines; audit charts; maintain active PT license.
  • Supervision & Professionalism - Oversee PTAs and nurse assistants; participate in on‑call requirements, case conferences, mandatory meetings; assist in orientation/training; maintain professional appearance.
Qualifications & Attributes
  • Licensure:
    Current Physical Therapist license in the state of employment.
  • Experience:

    Minimum 1 year as PT in an acute setting; home‑care experience is desired.
  • Skills:

    Ability to make sound PT judgments and solve situational challenges.
  • Communication:
    Create positive impressions with patients, families, physicians, and co‑workers.
  • Work Ethic:
    Detail‑oriented, observant, disciplined; prioritize and be timely.
Requirements

Valid driver’s license and auto insurance with your name as a listed driver. Knowledgeable on therapy best practices.

Document deadlines:
Subsequent, discharge, eval/recert/resumption visits within 24 hours; start‑of‑care within 48 hours.

Establish and follow plan of care within scope of practice for a Licensed Physical Therapist.

Health Qualifications & Physical Demands

Must meet physical position requirements. Evidence of current negative TB test/x‑ray required annually. Free of conditions restricting job functions. Risk for exposure to blood borne pathogens exists. Travel required.

Constantly (66–100%):
Reading, Speaking/Writing English, Computer/PDA usage, Hearing, Seeing, Talking, Stretching, Reaching, Walking, Sitting, Bending, Standing. Frequently (34–66%):
Driving, Lifting up to 50 lbs. Occasionally (2–33%):
Distinguish smell/taste.
